The following forms have been approved by WEA under section 44 of the Bulk Scheme.

Exporter Accreditation Application Form
To be completed by an exporter in order to apply for WEA accreditation.

Below are attachments to the Exporter Accreditation Application Form (Note: most attachments are already included in the application form):
Attachment A - Attachment cover sheet
This is to be completed for each attachment to the Exporter Accreditation Application Form.
Attachment B – Statutory Declaration for section 1.8
This is to be signed by the Chairman (or equivalent) in relation to the risk management systems in place for the accredited exporter. It must be submitted as part of an application for accreditation. For further information see section 1.10 of the Exporter Accreditation Application Form.
Attachment C - Summary and assessment of each executive officer
This is to be completed by the applicant for each executive officer and submitted as part of an application for accreditation. For further details see section 5.1 of the Exporter Accreditation Application Form.
Attachment D - Checklist for each executive officer
This checklist must be completed and signed by each executive officer and submitted as part of an application for accreditation. For further details see section 5.2 of the Exporter Accreditation Application Form.
Attachment E - Consent and National Police Check application form
Each executive officer is required to complete the Consent to Obtain Personal Information and the National Police Check Application Form.
The Consent and National Police Check application form for each executive officer must be submitted with an application for accreditation to WEA, do not submit the National Police Check application form directly to the AFP. Visit the AFP websitefor further information on National Police Checks.
Additional executive officers
Under section 5.4 of the Exporter Accreditation Application Form, details of each executive officer must be provided. The Application Form provides space for three executive officers. This form can be used if an exporter has more than three executive officers.

Attachment F - Risk management documentation
An applicant is required to submit the requested documentation as part of an application for accreditation. For further information see section 1.8 of the Exporter Accreditation Application Form.
Attachment G - Sourcing wheat
An applicant is required to include the information requested in this attachment as part of section 2.2 of the Exporter Accreditation Application Form.
